You can complete your diaper request online by completing the registration form below or by coming into the office on the days listed below. 

 

Below are two links to our registration forms, which provide us with the information we need about you and your baby. Please answer all questions truthfully and completely. This form provides us with vital information to apply for grants so we can continue to provide diapers. Rest assured that your information is protected, and we will never sell your information to anyone.

 

Please note that you can only request clothing once every six months.

After completing your registration form, click SUBMIT.

Next, visit our offices at 117 Park Avenue, Hamilton, OH 45013, on Wednesdays from 4:00-6:30p or Fridays between 11:00a -1:30pm.
